I noticed this the other day when I went to get healed. If you get a really good trainer car like the black one, the nurse will give this speech complementing you and the honor of healing your guys. I missed the part of it because I wasn't paying attention. Every time after that she'll say something like, "I'm glad to see you again *insert name here* will you have the usual?" If you say yes she'll say,"I'll be delighted/honored to heal you Pokemon." I don't know if that would be an unlockable but...
